Physical Examination

An updated Health History Form must be completed for each season a student participates in a sport. Any student-athlete who has tested positive for COVID-19 must submit a clearance note from their respective doctor (the note is good for two calendar years).

Physical examinations are Valid for 12 months from the last date of Physical Examination, with a normal Annual Health Questionnaire.
